Home and Community Treatment (HCT)​

At Rae of Hope, LLC, we provide compassionate Home and Community Treatment (HCT) services for children and adolescents experiencing emotional, behavioral, or psychiatric challenges that significantly impact their ability to function at home, in school, or within the community.

Our clinicians and Behavioral Health Professionals work collaboratively with youth, families, and care teams to provide intensive, family-centered support focused on emotional regulation, coping skills, behavioral stability, and healthy family relationships. Through individualized, trauma-informed care delivered primarily in the home and community, we help youth build the skills needed to improve daily functioning and reduce the need for higher levels of care.

Whether a child is struggling with significant behavioral challenges, emotional distress, or family conflict, Rae of Hope is here to help families strengthen supports, build resilience, and work toward lasting stability and well-being.

Who We Help

Children and Adolescents (Age Birth-20)

We support children and adolescents experiencing significant emotional, behavioral, or psychiatric challenges that affect their ability to function at home, in school, or within the community. Our HCT team works closely with youth, families, schools, and other providers to deliver coordinated, family-centered care that promotes stability, strengthens relationships, and helps youth develop the skills needed to succeed in their daily environments.

How HCT Services Help

Home and Community Treatment provides intensive support to help youth and families develop the skills needed to manage emotional and behavioral challenges in everyday environments.

 

Services may focus on:

• Emotional regulation and coping skills
• Behavioral stabilization
• Family communication and relationship building
• Problem solving and conflict resolution
• Strengthening daily routines and structure
• Building resilience and healthy coping strategies

 

Our goal is to help youth stabilize in their homes and communities while building the tools needed for long-term emotional well-being.

Where Services Are Provided

HCT services are primarily delivered in the home and community, allowing youth and families to work on skills in the environments where challenges occur.

 

Services may take place in:

• The child’s home
• Community settings
• School environments when appropriate
• Office-based sessions when clinically appropriate

 

Providing services in real-life environments helps youth and families practice and apply skills in meaningful and lasting ways.
