At Rae of Hope, LLC, we provide compassionate targeted case management services for children, adolescents, and adults experiencing mental health challenges that impact their ability to function successfully at home, school, work, or within the community.

Our case managers work collaboratively with clients, families, providers, and community resources to coordinate services, reduce barriers to care, and support stability and progress. Through a person-centered and trauma-informed approach, we help individuals access the supports they need to build independence, strengthen natural support systems, and improve overall well-being.

Who We Help
Children and Adolescents (Age Birth-18)
We support youth experiencing emotional or behavioral challenges that affect their ability to function at home, in school, or within the community. Case managers collaborate closely with families, schools, and treatment providers to ensure youth receive coordinated and effective care.
Adults (18+)
We support adults living with serious or persistent mental health conditions who may need assistance navigating services and maintaining stability in the community. Case managers help individuals access behavioral health care, housing resources, employment supports, and other essential services.
How Case Management Helps
Targeted Case Management helps individuals and families navigate the behavioral health system and connect to the resources needed for stability and recovery. Services may include:

• Advocating for clients within service systems

Our Approach
Rae of Hope follows a trauma-informed, strengths-based, and person-centered model of care. Each client works with a dedicated case manager who collaborates with the individual and their support system to develop and implement a personalized service plan.
